Okay, y'all. Time to bust out with those mad code skillz. I'm looking for a small submission form for Memeblog to make it quick and easy for people to submit their stuff. It would sit in the sidebar and the results would be emailed to a preconfigured address. The form would have four parts:
Field 1, text fill-in: Memename
Field 2, text fill-in: URL
Field 3, text fill-in: email
Field 4, drop down box: Type (Options would be Submission Reminder, Posted, Introduction, etc)
Fields 1 & 4 would be mandatory. Either 2 or 3 would be mandatory (both would okay but we need at least one of them for sure).
Can anybody help with this?

For form validation you can use Javascript to ensure the form has been filled in... or someone needs to write a super crazy PHP form handler with validation and postback ... I think JavScript is the way to go. I've got a load of validation stuff so can email it over
